Alternate Year Physical Form

This form must be completed and submitted on years opposite of having a completed physical.

Concussion Information & Acknowledgement Form

This form must be completed and submitted prior to a student participating in athletics.

Physical Form

The physical form must be completed every other year, with April 1st as the earliest day of examination. The date of the last physical the school has on file for your student can be found by going to Skyward under the health tab or you may call the high school office at (262) 626-8427, ext. 4127 during the school year.

John Underwood Presentation
August 6, 2012

On August 6, 2012 we have the privilege of having John Underwood come to KHS to speak to our students and their parents. Mr. Underwood is a nationally renowned speaker on the topics of substance use and its effects on the brain and body, and on training and conditioning. Some of our students had the opportunity to hear him speak last fall in West Bend and were impressed with his presentation. This year we are going to have our initial Athletic & Competitive Team Code meetings in conjunction with his presentations at KHS.
